I've seen the extruder in action.  It's a funnel wth a plug in the bottom.  Pull the trigger and a doughnut is born.
 
OTOH, if you atart waving silly money around, my mother in law will learn how to use a router and make you what would want. Some folks just confuse the "if you want it bad enough" price eih he cost of doing business. Especially on internet

They're building a new KK on my way to work. Thankfully it's on the wrong side of a busy road
 
It's not a "secret" so much as it's process that's not familiar to most folks.

A lot of foods are extruded. Snacks, cereals, pet foods, pasta, candies, etc. Many of them aren't even cooked, as the pressures involved create enough heat on their own.
