The Pickups are the Seymour Duncan P-Rails.  Single coil, P90, humbucker in one.  The extra switch is a mini 3 way toggle, Center position is humbuckers, forward is P90s, and back is single coils.

Is it a 4P3T switch that performs those functions for both pickups?  If so, where'd you get it.

You only need a dpdt on/off/on for that. Red and white to the middle terminal, black to one outer (and then to output), and green and bare to the other (and then to ground). Repeat with the other pickup on the other three terminals.

My bad.  I was thinking series/parallel/split.  I saw a 4 pole 3 position mini toggle and was considering it for a build, but not at $26.
