I once managed to cause a dead short in a 10kW 3 phase motor drive (long story).  An 8 gauge wire vaporized before the fuse opened up.

BTW, this is what that fuse looked like before the 'incident':

Well I took the tube holder out, luckly it was the rightmost one and for some reason is on its own PCB so it was very easy to get to. I hit all the solder points with some flux and an iron and put everything back together. Seemed to do the trick I couldn't get it to fail after I put everthing back together. The old man has a gig tomorrow so I guess that will be the real test.
