Wiring help - 3 humbucker, 5 way switch, volume, tone (push/pull), bass cut

I'm wondering if any of the wiring guru's can help me out with a good visual for what I'm trying to wire:

3 humbuckers
5 way switch
Volume (500k) with treble bleed
Standard treble cut tone (500k)  with push/pull for bridge pickup on
Bass cut tone (Reverse log 1M) (Reverend, G&L style)

I think it's a basic combination of the following, but would appreciate help putting it together visually.

For completeness' sake, are you using 4-wire or 2-wire humbuckers?  Also, standard Strat 5-way, superswitch, or import-style?  It doesn't change the way the stuff gets put together, I just want to whip up a diagram you can follow exactly.

Here's kind of a quick and dirty simplified way to combine the two mods together.  Note that the colors don't refer to anything, it's just to make a visual distinction rather than having black lines all over the place.  The wires coming from the humbuckers will be the "hot wires" - the ground wires go to ground, along with the bare wires.  If it's a 4-wire, you'll have to figure out which wires to tie off to keep the coils in series.  I can probably put together something better-looking, but this should get you started.
 
Thanks! These should be 2 conductor humbuckers, and standard US 5 way switch. But if they end up being 4 conductor, I know what to do with the other wires anyway.
