Is this for a strat? If so, here's what I have on my own strat - it gives every possible combination (except neck and bridge in series, no middle), looks stock, and the only 'dead spot' is in series, position 3. The "blend pot" is just the standard 500k pot with a push-pull.
Another good option for your super switch is the five-tone tele diagram below.
One other thing you might consider, and probably what I would do if next time: Just figure out what five sounds you need the most and wire the super switch to give you those. On a strat, maybe just for example you never use 'middle only' or 'bridge only' and those could be replaced by 'neck / bridge series out of phase' and 'neck / middle in series in phase' just for instance. You could wire up a switch like that no prob.

Neck and bridge in series or neck and bridge in parallel would be very noisey...not hum canceling...i presume it is an SSS guitar. If you had a HSH you could do this with hum canceling! Check out the attachment.
