The cost varies greatly depending on how intricate a design you want and the materials used, you pretty much have to get quotes. I'm shopping around myself, and seems like you're around US$75 for something like the one shown if you're using atypical font(s) for your script; more $$$ for real graphics work...

If you check eBay, you can find some set design work in various cover configs for $30 or less, but most of that is ordering from Taiwan/Hong Kong and based on my past experieces, Good Luck!
