Wiring diagram request

Hi, elecronics gurus.

I would be most grateful to anyone who can supply  a diagram that will serve the following purpose:

- Two single coil pickups - one Phat Cat, the other a Quarter Pound Tele p'up with a coil tap.

Desired configuration:

Four way pickup selector switch, with these combos:

Neck only
Neck/bridge, parallel
Neck/bridge, series
Bridge only

And a separate mini toggle for the coil tap on the Quarter Pound.

One volume, one tone, output jack.
