Tele control plate with vol, tone, TOGGLE

Does anyone know where I can source tele control plate that have 3 holes (1 vol knob, 1 tone Knob and a 3 way toggle) NOT the traditional 2 holes and blade slot?
any help would be MUCH appreciated!

These guys have blanks that you can perforate any way you'd like, but they don't appear to be metal. If that doesn't matter, there you go. If you'd rather something stainless/polished, it would appear the Callaham solution Black Dog mentioned is the only other game in town.

Those Guitar Fetish blanks are metal, Cagey.  They also have 'em in chrome and gold.
