Anyone ever seen this before?

I ran across this last night, and I think I might be in love. I've got two question though. 1) Does anyone know what kind of guitar this is? and 2)is it a carved top?

Headstock has a large H, can't make out any of the letters after it.  I did check on headstock pictures of Hofner, Hohner, Heritage, Hondo, Harmony and it looked like none of those.
It looks old, or it's been reliced as such.  Body appears to be Tele custom with 2 Humbuckers, Bigsby vibrato, surface mount midi/piezo unit.
