I believe that since he wants the strings further from the neck, he wants the headstock to be raised. In my opinion, the shim should be placed on the other side of the heel, far away from the pickup.

Sorry for the crappy editing but I think you should run it like that otherwise you would not connect  any pickup to hot in the 4th position and have no sound. Maybe you could replace the pink wire by a small resistor to tune your low pass filter.

The preamp you selected do exists too for passive pickups http://www.warmoth.com/Active-Bass-Preamp-passive-voiced-3band-STC-3P-P1462C810.aspx. It seems better to me as it's meant to 2pickups basses. It as a blend volume control that the music man preamp doesn't have.
